OUR FACILITY
Our Skilled Care Unit is a Medicare-certified 40 bed facility that provides care for patients with a wide variety of medical conditions. We provide comprehensive rehabilitation care to our patients through specialized equipment and facilities that include all private rooms, a customized therapy gym, an activities of daily-living apartment and a communal dining/activity room.
OUR TEAM
The Trinity Skilled Care unit offers patients an interdisciplinary treatment plan focused on healing. Because each patient’s needs are different, our integrated skilled team identifies the patient’s priorities and creates a customized, comprehensive treatment plan.
Physical therapists focus on restoring movement and function while Occupational therapists work with the patient on recovering skills needed for everyday living.

Ask for our Admission’s Coordinator, who can assist in helping you with the appropriate services for your needs.
Our speech and Language Pathologist works with patients that have communication, cognitive, and swallowing deficits. Our team is rounded out with Registered Dietitians for nutritional management and a Discharge Planner that coordinates with community resources to ensure a well planned and safe discharge back to home or other appropriate facility when needed. Spiritual care is also provided by our pastoral care staff to assist patients and families with emotional support and healing.
OUR GOAL
The goal of our Skilled Team is to help patients become as strong and independent as possible. Communication between the patient care team and family members during the inpatient stay will help facilitate a smooth transition home. To meet this goal, as a team we work with the patient and family to facilitate patient and family education, discharge planning, and support of community resources for the transition back to home.
