Acute Rehabilitation Unit
Our Rehab Unit is a Medicare-certified 20 bed facility that provides care for patients with a wide variety of medical conditions. We provide comprehensive rehabilitation care to our patients through specialized equipment and facilities that include all private rooms, a customized rehab therapy gym, an activities of daily-living apartment and a communal dining/activity room.
The Trinity Acute Inpatient Rehabilitation unit offers patients an interdisciplinary treatment plan focused on healing. Because each patient’s needs are different, our integrated rehab team identifies the patient’s priorities and creates a customized, comprehensive treatment plan.
Leading the team is a physiatrist, a physician who specializes in physical medicine and rehabilitation. Dr. Milton Swaby is a Board Certified Physiatrist who oversees the rehabilitation techniques, medical management, and methods of treating and preventing impairment. The patient care team also includes specially trained rehabilitation nurses who manage the various aspects of the patient’s care. Physical Occupational and Speech therapy provide patient centered goals and treatment that are tailor-made for the families and patients to gain their highest level of independence possible.
Physical therapists focus on restoring movement and function while Occupational therapists work with the patient on recovering skills needed for everyday living. Our Speech and Language Pathologist works with patients that have communication, cognitive, and swallowing deficits. Our team is rounded out with Registered Dietitians for nutritional management and a Discharge Planner that coordinates with community resources to ensure a well planned and safe discharge back to home or other appropriate facility when needed. Spiritual care is also provided by our pastoral care staff to assist patients and families with emotional support and healing.
The goal of our Rehabilitation Team is to help patients become as strong and independent as possible.Communication between the patient care team and family members during the inpatient’s stay will help facilitate a smooth transition home. To meet this goal, as a team we work with the patient and family to facilitate patient and family education, discharge planning, and support of community resources for the transition back to home.
Our Rehabilitation Team specializes in medically complex patients. We care for patients with a variety of conditions, including Stroke Recovery, Neurological Disorders such as MS, Parkinson’s and Guillain Barre, Orthopedic injuries, Joint Replacements and general Debilitation. We also care for conditions such as Spinal Cord injuries, Traumatic Brain Injuries as well as Amputations with Prosthetic Training.
To be Eligible for admission to Acute Inpatient Rehab you must:
- Require 24 hour Nursing care
- Require two or more therapy needs (physical, occupational, speech)
- Be able to participate in three hours of therapy, no less than 5 days a week
- Have the potential to improve function
